Medical Information Systems

Lectures

1. Organization of the Czech healthcare system and healthcare delivery; types of healthcare information systems; relevant legislation.
2. Information systems: characteristics and properties; classification and types; information system architecture; distributed information networks; data security from multiple perspectives; database backup and archiving.
3. Databases: basic concepts and architectures of database management systems; database models; query languages.
4. Medical information systems; healthcare data processing; core reference tables and coding systems; design and implementation of a hospital information system (HIS/NIS).
5. HIS (NIS) and its components I: clinical information system, outpatient information system, dental information system, transfusion information system.
6. HIS (NIS) and its components II: radiology information system, pathology information system, functional diagnostics information system, laboratory information system.
7. HIS (NIS) and its components III: pharmaceutical information system, pharmacology information system, microbiology information system, rehabilitation and spa care.
8. HIS (NIS) and its components IV: catering services, transport/logistics, Master Patient Index, management information system.
9. Data and communication standards: general standards (CSV, XML, JSON, DICOM, REST, SOAP) and healthcare standards (Czech DASTA, international HL7, FHIR, CDA).
10. Health insurance and healthcare billing: registration of procedures according to VZP code lists; DRG; reimbursement decree.
11. eHealth: interoperability, telemedicine, core registries, ePrescription, EHR, IHE.
12. Information system development: development phases; waterfall vs. agile approaches; documentation; version control.
13. Artificial intelligence and its applications in medicine.

Computer Labs

1. Fundamental concepts in computer science.
2. FreePascal/Lazarus: principles, structured programming, introduction to the IDE.
3. FreePascal/Lazarus: console application development.
4. FreePascal/Lazarus: basics of GUI applications; LCL library.
5. Professional microbiology information system (HPL).
6. Telemedicine: hands-on demonstration of capabilities.
7. FreePascal/Lazarus: components and events; returning values.
8. SQLite: basic commands; database creation.
9. FreePascal/Lazarus: connecting database components to SQLite.
10. FreePascal/Lazarus: working with database tables; master-detail relationships.
11. FreePascal/Lazarus: multi-form applications; reference tables.
12. FreePascal/Lazarus: dynamic generation of components.
13. FreePascal/Lazarus: file handling. 

 

Team project

Creation of an assigned information system module and its presentation.

Entry knowledge

The student should be able to explain fundamental principles of data processing, should know basic terms of healthcare processes and should be oriented in basic knowledge of database systems. Generally, knowledge of mathematics on the level of Bachelor study is required.

Rules for evaluation and completion of the course

up to 30 points from computer labs (1 individual project, 1 test)
up to 70 points from final oral exam
The exam is oriented to verification of orientation in terms of medical information systems and ability to define a structure of medical information systems.
Computer labs are obligatory. Excused absence can be substituted.

Aims

The aim of the course is to inform students about principles of specification of medical data regarding its processing by diagnostic systems and medical information systems, provide information about structure of medical data representation, its role in diagnostic systems and concepts of medical information systems.
The student will be able to:
- describe basic structures of medical information systems,
- explain fundamental terms of electronic health records,
- describe principle of basic methods for processing of data in information systems,
- discus advantages and disadvantages of the methods,
- work with clinical information system.

1. Organization of Czech health service, data being processed in a health care system
2. Choice of a correct database, problems of large databases' processing
3. Differencies between various health care information systems
4. Hospital information system, build-up of HIS - one of the most difficult tasks of medical informatics
5. HIS with centralized and with distributed databases
6. Information coding, National Health IS master files (coding lists), medical information coding, nomenclature, WHO coding systems
7. Electronic patient record, alternative free text processing
8. Communication between different IS, data standards, Czech standard for medical data transmission, standard HL-7, standard DICOM
9. General rules of large HIS build-up
10. Data security from various points of view, database backup and archiving
11. Use of Internet for medical informatics, Czech project IZIP, Document Imaging Systems
12. Planning systems, Scheduling, electronic questionaries in medical informatics
13. Management and decission support information systems
