ST-segment elevation myocardial infarction (STEMI) is a serious acute condition requiring rapid initiation of treatment. The main goal of therapy is to restore blood flow in the affected coronary artery as quickly as possible, most commonly by percutaneous coronary intervention (PCI). However, reperfusion therapy may be influenced by several factors, including platelet activation, microthrombus formation, and microcirculatory dysfunction. Therefore, platelet inhibitors represent an important part of the treatment of patients with STEMI. This diploma thesis focuses on the assessment of myocardial reperfusion in patients with STEMI treated with reperfusion therapy including platelet inhibitors. Myocardial reperfusion was assessed indirectly using changes in ST-segment elevation on the ECG recording. In the practical part, patient data were analyzed, with ST elevation evaluated before the initiation of treatment and subsequently after 90 minutes. The data were further supplemented with selected clinical characteristics, infarct localization, and risk factors. In the analyzed group, a decrease in ST elevation after treatment initiation was observed in most patients. This finding can be interpreted as a sign of a favorable response to reperfusion therapy and possible improvement in blood supply to the ischemic myocardium. However, in some patients, a sufficient decrease in ST elevation was not achieved, which may be related to persistent microcirculatory dysfunction, a more severe extent of ischemic injury, or other clinical factors. The obtained data also suggest that the response to reperfusion therapy is individual and may be influenced by the patient’s overall clinical condition, the result of the PCI procedure, and the presence of risk factors.

Studentka prezentovala výsledky své práce a komise byla seznámena s posudky. Prof. Penhaker se doptal na použité statistické metody. Ing. Němcová se doptala na typ léčby a ověření normality dat. Ing. Plešinger se doptal na anlýzu vztahů mezi datami. Studentka obhájila diplomovou práci s výhradami a odpověděla na otázky členů komise a oponenta.
